Online Payment Form

You can pay with a credit card or PayPal.
You can choose to pay a full or partial balance and request setting up a payment plan.
The payment will be processed and applied to your account at a later time.
Patient Portal
Payments through the Patient Portal will be applied directly to your account.
It requires your previously having set up a log in and password to that specific section of our website.
Online Chat/Venmo
If we are online, you can pay towards your balance from within the chat window or through Venmo.
